Job Purpose:
Fundamentally different from a standard domiciliary care service, the focus will be on helping older people “to do for themselves” in their own home rather than “doing to or on” them.

Responsibilities : 

To prepare for the return home to include basic shopping, collecting prescriptions and ensuring the home is sufficiently heated and comfortable.

To actively encourage and work with the person to restore the confidence and skills they require for independent living in their own home,   including, but not solely, working with the person on household tasks, kitchen practice, shopping, laundry, cleaning, preparation of meals and  locating social activities.

To support the person to become familiar with and settle into their home environment and work with the person to promote independence in transfers, and indoor and outdoor mobility.

To work with the person and restore skills in looking after their own personal care.

To co-design, monitor and evaluate Person-Centred Active Support Plans with people by establishing a baseline and identifying agreed goals, strategies and improved outcomes.

To assist / support service users to take medication in accordance with organisational policy.

To communicate with carers and family members to promote the ethos of active support and secure their assistance with planned programmes/activities to restore the independence of their family member.

To be aware of individual service user’s changing needs and to communicate these to the Project Manager and participate in individual review meetings and team meetings.

To communicate with Trust personnel, GP’s and other agencies as appropriate.

To participate in training as required.


Essential Criteria:

At least 1 years paid experience in a caring role

GCSE (or equivalent) standard of education

Relevant qualification e.g. QCF Diploma in Health and Social Care, Level 2/3 or equivalent OR willing to complete within 12 months of commencement in post
